You will need to sign in to your USA Hockey profile page.
Go to www.usahockey.com
Click on the "OFFICIALS" link.
Click on the "REFEREE LOGIN" link.
Enter your user name
Enter your password
Once you are logged in, Click on the "FILE AN INCIDENT REPORT" link.
Click on the "REPORT INCIDENT" button; It is a large green button.
It is important that you fill this report out as completely as possible, including a DETAILED description of the incident.
